Welcome to the Second Stork Community Impact Survey. Please tell us how we are doing.

Second Stork continually seeks to improve your experience with us and measure our impact on the lives of parents in need by providing immediate access to items that will keep their newborns safe and healthy.

As a vital partner to our operation, your input is essential to the work we do. The information we collect is used to inform our board decision making, to inspire our volunteers and donors, and to bring pertinent information to the attention of grantors who help fund and sustain our work.

This survey should take five minutes or less to complete. Responses will be anonymous when communicated publicly. Your response by Friday, October 4 is appreciated.

Second Stork's mission is to provide families in need with essential newborn care items to give their baby a safe and healthy start. Second Stork relies on social workers and nurses to determine when a family does not have the means to secure the essential items to care for their newborn upon discharge, and to only give to the family the items that they lack. All items from Second Stork are given to families as packaged by Second Stork.

QUESTION 12: Second Stork has expanded into 5 states (MN, WI, ND, SD, and IA) and serves over 100 hospitals. We are looking to recruit more volunteers to "adopt" their local hospital. Does your hospital have a newsletter where we could place a request for volunteers who occasionally travel to the Twin Cities and would be interested in picking up an order at our St. Paul warehouse? Do you have any suggestions of local clubs, churches, or service organizations that we could reach out to? We appreciate any suggestions!
